4. Provide the bookmaker's complaint reference number if you have one.
5. Select the bookmaker from the list and provide your username or account number with the betting company.

6. Select the method of betting - online, over the counter, or other.
7. Describe the situation, including necessary dates, amounts, receipts, account numbers, bets, transaction codes and other pertinent details if available.

8. Attach screenshots, scanned images and other files as necessary.
9. Confirm agreement with the organization's rules and click ‘Submit claim’.
